Noticed joint aches after a week on low‑dose letrozole – anyone else?

5 mg twice a week to keep estradiol in check while I’m on TRT. After about five days I began feeling a dull ache in my knees and shoulders, especially in the mornings. It’s not painful enough to stop training but it’s noticeable and makes me wonder if it’s a common side effect or just coincidence.

I’ve read the description mentions joint pain as a possible issue, but I’m keen to hear if others have felt the same and whether it tended to fade with continued use or if they adjusted the dose. I’m also thinking about getting some basic bloodwork done to see if anything else is shifting, but I’m not sure what markers to ask for beyond the usual estradiol and testosterone. Any advice on how to monitor this safely would be much appreciated.

No worries if you haven’t tried it yourself, just keen to learn from the community. Good on ya for sharing your experiences.

  • reid_f: I've dabbled with letrozole for a couple of months on the same dose you mentioned and the joint aches were pretty consistent at first, especially in the mornings, but they got quieter after a month or so, maybe as the body settled into the lower estrogen levels, or just the chronic inflammation from training got a little less of a slap. I would have gotten a quick panel in the first week, estradiol, testosterone, LH, FSH, and a quick CRP or ESR just to makeDisclaimer that i’m not a doc, so if yo
  • researcher_liam: Thanks for the info, reid_f. I'll get a panel in the first week for estr corridors and, as you suggested, LH, FSH and CRP. I’ll see if the joint aches quiet down after a month like you said or if they persist. Do you have a go‑to dose adjustment or a breakpoint where you’d scale it back? Also, would you add any anti‑inflammatory supplements to help the joint feel better?
  • retiredcole: Tbh I’ve felt that same morning tightness when I first started letrozole, first week or so, but it did get less after a month or two. I also stuck to a quick panel early on – estradiol, testosterone, CRP – and saw pretty stable levels. If the pain worsens, dropping the dose or adding a joint‑support supplement could help, but talk to your doc first.
