NPP Injections: My Experience with Low Testosterone and Injection Pain

I started NPP last year for lean mass gains, injecting once every three days. A few weeks in, my testosterone labs dropped below 200 ng/dL. I did a second test a month later and it stayed low.
Anecdotally, the only thing that changed was the steroid; nothing else in my routine. I began soaking the injection sites in warm compresses beforehand and used a 25‑gauge needle, but the pain was still sharp, especially on the thigh. I tried rotating sites, but the soreness stuck around.
I added a short course of naproxen after the injection and the discomfort eased. I also started tracking my mood and energy; I felt more lethargic and had a mild night‑time tremor. My doctor advised a testosterone replacement if levels stayed low.
I’m thinking about cutting the dose to 50 mg every four days and monitoring labs every six weeks. Anyone else hit low T or injection pain with NPP? What’s the safest tweak you’ve tried?

- brandon_p: I’ve been in the same spot. With my low‑dose NPP (about 25 mg every 3–4 days) the pain on the quadriceps just didn’t go away, even with 25‑gauge needles and warm compresses. The only thing that helped was a 12‑hour naproxen after the shot – I smelled the stomach upset, but the soreness dropped from 8/10 to about 3/10. I didn’t notice my testosterone dropping that low; I were more in a pre‑stage cycle, so I kept an eye on labs and didn’t cross the 200 ng/dL line. If you’re still low, maybe try a
- sleep_coastal: I’ve been doing the 12‑hour naproxen just like you, softer but still a stomach hitch. I’ll try the 26‑gauge needle and the 24‑hour rest you mentioned next week. One thing I wondered: after you give a site a full day off, did the pain keep dropping or just stay steady? Also, what’s your lab cadence when you keep the dose that low?
