Trying Pentosan Polysulfate for Post-ACL Knee Pain – Dosing and Effectiveness?

I had an ACL reconstruction two years ago and the rehab went pretty well but the knee still chafes after a workout or even sitting for a long time. I came across Pentosan Polysulfate – the same thing used for interstitial cystitis – and some chatter about it helping with osteoarthritis and cartilage repair. The oral protocol seems simple: 100 mg three times a day, so about 300 mg total.

I’ve started that now, just after my normal breakfast, lunch, and dinner. My first week the only thing I felt was a mild stomach upset – maybe the GI side effect I’ve heard about – but otherwise no headache or hair issues. After a month I noticed a slightly smoother feel when I bend the knee, and the soreness after a run dropped from a 7/10 to a 4/10.

I’m still watching for any bruising or bleeding signs and haven’t had any yet. Anyone else in the community using it or a similar dose? How long did it take you to see benefits, and did you combine it with anything else for joint health?
