TB4-Frag before or after physio work on a cranky Achilles – whats working for ppl?

Ngl my left Achilles has been a thorn in my side for like 8 months now. Mild tendinopathy that flares up after longer runs or hills. Been doing heavy slow eccentric work 3x a week and my physio does ASTYM plus dry needling. Started running TB4-Frag about 3 weeks ago, 2mg subq, and im trying to figure out timing.

Ive been doing it right before i leave for physio which means maybe 30-45 min before the deep tissue work. The idea was get blood flow going and maybe help the remodeling window after ASTYM. Honestly idk if i can even feel it acutely but the past 2 sessions my achilles has been less angry the next day. Could just be coincidence tho.

Curious how others are timing it. Do you pin before the session to try to catch the inflammatory window, or after when the microtrauma is already done? And has anyone split the dose around the session, like half before half after? Also wondering if i should be moving it to evenings on non physio days vs keeping it consistent every day. Bloodwork has been clean so far.

  • casey613: For what it's worth, I ran TB4-Frag for a cranky ankle thing last year and ended up doing it post-session rather than before, mostly because I noticed the limb felt a bit "heavy" or weird during the actual soft tissue work if I pinned close to it, though that could absolutely just be me overthinking it. Some people split it morning and night around the session days, but I just kept it simple at 2mg subq about an hour after physio and stuck with daily timing on off days. Consistency seemed to mat
  • grinder265: Ngl that "heavy" sensation makes sense, I think I mightve been a little numb to it cause the ASTYM is already so intense that any subtle difference gets masked. I'll try shifting to about an hour post-session for the next couple weeks and see if the next-day response changes. You mentioned sticking with daily timing on off days, were you doing 2mg every day or were you doing it 5x a week? Im currently doing 7 days straight and not sure if i should be cycling off at some point.
  • grinder265: Appreciate that, ngl i hadnt even thought about the "heavy limb" thing during the work itself. That kinda makes sense tho, if its pulling blood flow somewhere while theyre trying to mobilize it could feel off. Ill try flipping to post-session for a couple weeks and see if i notice anything diff. Daily timing on off days is what im doing already so thats easy to keep.
