Rapamycin + wound healing concerns, how long after a dose before elective surgery is safe?

Got a procedure coming up in about 6 weeks, nothing crazy but enough that i want to be smart about it. Been on low dose rapa weekly for the longevity angle, 5mg once a week, and i keep seeing impaired wound healing flagged as a real side effect since it tanks mTOR right when your body needs it for tissue repair.

For anyone whos run rapa long term and then had surgery or a significant injury, how did you time it? Did you hold the dose for a set period before and after, or just ride it through? The autophagy benefits are why i started so skipping weekly doses for weeks at a time kinda defeats the point for me.

Also curious if anyone has tried bridging with something like BPC or TB-500 during the peri-op window to offset the healing slowdown, or does that just muddy the picture. Ngl a little nervous about stacking unknowns when the surgery date is already locked in. Not looking for a protocol, just real world timing experiences.

  • student_cora: Honestly 6 weeks out i would just hold the dose for the week before and the week after, then ease back in. Autophagy benefits don't vanish from skipping 2 doses, and healing is the bigger risk in your face right now. Check anastasiou or attia podcasts too, theyve covered this exact thing.
  • grinder265: Yeah 2 weeks off isnt the end of the world, i was probably overthinking the autophagy part. I forget that mTOR suppression is more of a chronic thing than a single missed dose thing. Ill hold the week before and after like you said, been meaning to listen to that attia episode anyway so ill queue it up. One thing tho, did you actually go thru this yourself or is it more of a general read on the timing?

The autophagy window builds back up fast so skipping a couple doses probably doesn't undo the whole protocol. Surgeon buy-in is huge though 🔬

  • grinder265: Honestly that was supposed to be MY post you quoted back at me lol. But to answer what you're asking, yeah im holding 2 weeks pre and 2 weeks post then restarting. My surgeon didnt know much about rapa specifically so i showed him the immunosuppressant literature and we landed on that window together. Might bridge with BPC subq around the procedure but havent decided yet.
