BPC-157 oral vs injectable for a grumbling supraspinatus, what finally worked for you

Rotator cuff has been nagging me for about 4 months now, supraspinatus tendonopathy confirmed on imaging, and I have been rotating between oral BPC and subq injections to see what actually moves the needle. Started on oral arginate salt at 500mcg twice daily and it helped the general achiness but the sharp catch on overhead pressing was still there, so I switched to 250mcg injectable near the shoulder plus 500mcg oral in the morning. The combo seemed to make a real difference after week 3, less morning stiffness, and I could load lateral raises heavier without that pinching feeling.

I have also been layering in TB-500 once a week and keeping rehab consistent with slow eccentrics and scap work. Curious if anyone here has run a similar BPC oral plus injectable split and how long you ran it before tapering. Also wondering if topical diclofenac on the deltoid before physio is worth keeping in the rotation or if it blunts the healing response, because I have read mixed things on NSAIDs around tendon recovery. Steady lah, just want the shoulder to stop being the weak link in my training.

  • elle_z: Haven't done the split you're running but I did 8 weeks of subq near the tendon plus slow eccentrics for a similar issue, and honestly the eccentrics were probably doing 70% of the work. BPC just seemed to take the edge off the morning stiffness for me. On the diclofenac question, my physio was pretty against using it before sessions because she said it can dampen the inflammatory signal you're trying to keep for remodelling.   • casey613: Honestly the eccentrics point tracks with what I felt too, the heavy slow stuff felt like it was doing more than any of the peptides by week 2, BPC just kind of smoothed out the spikes between sessions. The diclofenac thing is interesting though, my physio never flagged that and I've been using it pre-session for maybe 6 weeks, will probably swap to ice going forward and see if I notice a change in how the tissue responds the next day.   • quiet_ironman: Good call on the eccentrics, I kinda slacked on those once the BPC started feeling like it was helping. Bringing them back in this week, slow tempo on banded external rotations mostly. Diclofenac timing is interesting, my physio said the opposite basically, said a short course early on is fine to break the pain cycle so you can actually load it. Go figure. Haven't tried TB-500 yet honestly, that's the next thing I'm mulling over. Curious what others have run alongside BPC for stubborn tendons
  • casey613: Ha, physios really do split on the diclofenac thing. My guy leans more toward using it as a window to actually load the tendon, kind of what yours is saying I think. Going to keep it on session days and see if I can push the eccentrics a bit harder without that guarding feeling kicking in. TB-500 has been steady for me, once a week subq, nothing fancy, just feels like blood flow to the area is better and recovery between sessions is less of a grind. If you try it, give it at least 4 to 6 weeks
