The Pharmaceutical Journal has a good overview of what's in the pipeline after semaglutide and tirzepatide, with MariTide standing out because of the dual agonist/antagonist mechanism and the monthly dosing.
Honestly, that part is what caught my eye. Monthly would be huge for adherence (no?), because weekly injections are still a barrier for a lot of people in my circle.
I am skeptical though. The phase II weight loss numbers look promising on paper, but until I see the full side effect profile and a bigger trial, I would not get excited. Fwiw, the gastrointestinal tolerability with this class has been the real limiting factor, not efficacy.
Has anyone here been following the MariTide data closely? Curious how the dosing intervals compare to what most people are running now.

- quiet_hrv: Monthly would definitely help with sticking to it, that part I agree with. The mechanism is what interests me actually more though. Amylin agonism on top of GIP antagonism is a different angle than what tirzepatide does, so at least it is not just another me-too GLP. I have only skimmed the phase II so far... GI dropouts were not terrible but not nothing either. Curious whether the monthly schedule ends up making side effects feel worse per shot since you are basically getting a bigger load ea
- night_optimizer: Yeah that dosing question is a real one, and I have not seen anything super clear on it either. It kinda makes sense that a bigger hit per shot could mean rougher peaks, but without seeing actual data I'd just be guessing. Worth keeping an eye out when more phase II stuff gets published 🙂
- amber75: Same here, the peak/trough math is what I want to see. For me on a weekly GLP-1, the day after injection is always rougher GI-wise, so a monthly version with a bigger bolus makes me a little nervous, you know? Have you looked at the CagriSema data at all? Curious how the monthly dosing numbers compare.
- amber75: That bigger load per shot question is exactly what I keep coming back to too (fwiw, I have been poking around for pharmacokinetic data on the monthly interval and it is pretty thin so far). The amylin piece is interesting to me, more or less because it touches satiety from a different pathway, so for people who plateau on tirzepatide it could matter, right? But you are right, it is not a me-too.
