BPC-157 in sports medicine, finally a paper that tries to sort the mess

Saw this one land on PMC, it's basically a scoping review pulling together what's been published on BPC-157 in ortho and sports med settings, including the human case reports floating around. They walk through tendon, ligament, and bone healing studies, mostly animal, and flag how thin the actual clinical evidence still is even with all the buzz in the rehab world.

What got me is they don't bury the lead, they straight up say human data is sparse and a lot of the mechanistic stuff (NO signaling, FAK pathway, angiogenic stuff) is rodent work. That's been my gut take for a while. The signaling cascade claims get repeated like gospel online and most of it traces back to the Sikiric lab in Zagreb.

Anyone else read through this? Curious if the case report stuff on Achilles and quadriceps recovery they cite actually convinced anyone or if it's still pretty much "could just be placebo" territory.

  • quiet_ironman: I read that review too and it lines up with what I’ve seen in the gym. I tried a short course of BPC‑157 for a nagging elbow tendon issue last winter, just a low dose subQ a few times a week. For me the soreness eased after about ten days and I could lift heavier without the usual ache, but I’m honest enough to say it could have been placebo or just the rest I accidentally took while waiting for the vial to arrive. The paper’s point about most mechanistic data coming from rodent labs matches my
  • frugalkyle: Yeah, yeah. Same elbow tendon story here, subq a few weeks on and off. Soreness did seem to ease quicker than usual but like you, i can't really say it was the peptide and not just backing off heavy volume for a bit. Ngl, the rodent mechanism stuff is cool to read but it doesn't really translate to "this is what happens when a 45 year old desk jock uses it on his lateral epicondyle."
  • grinder265: Ngl the "backing off volume" confound is exactly what kills me about these anecdotal recovery stories. Like you ease up on training AND introduce the peptide at the same time, how do you even begin to separate those. That's my whole issue with the Achilles case reports too, every one of them involves reduced load alongside the BPC. The lateral epicondyle thing is interesting tho, did you run it bilaterally or just the affected side?
  • grinder265: Ngl ten days tracks with what I hear most often from people who claim it hit for them, that week to two week window. The subQ a few times a week is basically the standard everyone lands on anecdotally. Did you run any kind of loading phase or just straight into the low dose, and how long was the total course? I always wonder if people who claim it works are just inadvertently giving themselves more rest than they realize, same issue with your vial wait time point. Easy trap to fall into when yo
