Anyone looking at CagriSema vs tirzepatide for a next cycle?

Been on tirz for about 8 months now, down roughly 22% bodyweight, but im starting to plateau hard. Last 6 weeks the scale barely moved even with keeping protein up and training 4x a week. My HRV has actually improved a lot which is nice but the fat loss just stalled out.
Started reading about CagriSema (cagrilintide plus semaglutide) and the mechanism makes sense to me on paper. Amylin hits satiety through a totally different pathway than GLP-1 so the idea of stacking both seems like it could break through a plateau that a single receptor agonist cant. The profile page here mentions potential 20-25% weight loss which is in the same ballpark as what people get on tirz, maybe a touch more.
My question is whether anyone has actually run cagrilintide on top of a GLP-1 they were already on, or switched over to CagriSema from tirz or standalone sema. Curious whether the appetite suppression felt different, like more physical fullness vs the GLP-1 "food noise just gone" thing. The GI side effects sound rougher with the combo though. Nausea on sema was bad enough the first month, dont really want to repeat that experience 😅
Also wondering about titration. The listed dose goes up to cagrilintide 2.4mg plus semaglutide 2.4mg weekly, but nobody jumps straight to that right? Seems like youd want to ramp each component separately.

- patient_codes: I’ve actually tried stacking a low dose of cagrilintide with my semaglutide for about four weeks when I hit a similar stall. The feeling was different – less of the “food noise just gone” calm and more a noticeable physical fullness after meals, like my stomach sensed volume faster. Nausea did kick in harder the first few days, but I split the dose and took it with a small snack which smoothed it out. My weight started moving again after week two, though I kept my protein high and lifted four t
- rachel_ketogen: Thats super useful, thanks for the actual experience. The "physical fullness faster" thing is exactly what id hope for since the food noise side already feels handled by the tirz. Two questions, did you drop the tirz dose at all when you added the cagrilintide or just kept it flat, and what dose of cagrilintide did you start at? I dont want to stack full agonist doses and cook my stomach 😅
- weekendnate: Yeah splitting the dose and having a few bites of food first made a big difference for me too. Interesting that you got the fullness signal more than the quieting of food noise, that tracks with amylin working through the gut-brain axis instead of the central appetite centers. Out of curiosity what timing did you land on, both shots on the same day or staggered?
- rachel_ketogen: Yeah they were both on the same day, CagriSema in the morning and tirz in the evening. Did that for 2 weeks then moved CagriSema to night too just to keep it simple and it felt the same to me honestly. Did you notice the satiety signal hit faster on the combo or was it more of a slow buildup? Mine was pretty fast, like within 30 min of eating i was full in a way tirz alone never did for me.
