GLP-1 compounders still flying under the radar even after the pricing deal

This BioSpace piece lays out how compounding pharmacies are still operating in a gray zone months after the big pricing agreement. The interesting bit is that the loophole around "personalized" copies has not really closed, and a lot of these operations are still shipping at scale.

That tracks with what I have been noticing on forums and in my own circle. People keep finding sources that look professional, with telehealth consults and tracked labs, but nobody can really tell you what is in the vial. The pricing deal helped some folks, but if the compounded supply chain is still this murky, the safety gap is the same.

What I keep coming back to is third party testing. If you are going to use a compounded GLP-1, a COA from a real lab should be non negotiable. Has anyone here actually gotten one back that matched the labeled concentration, or are most still taking it on faith?

  • weekend_experiment470: On the COA question, what I have seen personally is that the ones that come back from a real accredited lab (not the in house ones the pharmacies put out themselves) have usually been within a reasonable range, but rarely bang on the label number. For me it was something like 8 to 10 percent off the stated mg/mL, which is not terrible but it does mean your actual dose is a moving target.   • busyliam: Yeah the moving target thing is real, and for GLP-1s especially where people are titrating up based on side effects and appetite, being 8 to 10 percent off actually matters quite a bit na? Like if you think you are stepping up to 0.5 but you are really getting 0.55, the nausea profile changes. The in-house COAs I have personally seen are basically marketing material only, has anyone here actually compared an independent third party result against the pharmacy one for the same batch?
  • dita_cycles: Yeah that tracks with what I saw when I had two vials from the same supplier tested, they came back at like 92 and 96 percent of label, which is probably fine for most people but not great if you are titrating on small dose changes. The accredited COA route is really the only thing that makes sense to me, the in house paperwork tells you nothing.
