BPC-157 arginate oral for a cranky posterior tibial tendon, anyone running it longer than 6 weeks?

Ngl my posterior tibial tendon has been barking at me for like 3 months now. Started as a dull ache after long runs and now it's a sharp thing if I push mileage past 40 miles in a week. I've done 2 full rounds of injectable BPC at the standard 250-500 mcg range and it helped, tendon felt calmer within a week or so, but the second I stop and ramp mileage back up it comes right back.
Thinking about switching to the oral arginate salt since i'd rather not pin daily for months and i've heard the gut/organ angle might give some systemic support too. Dose range i'm looking at is 500 mcg to 1 mg once or twice a day based on what i've read.
Curious if anyone here has run oral BPC for a stubborn tendon for longer than 6 weeks straight. Did it actually hold up or did you feel like it wore off? And did you layer anything else, like TB-500 or even just topical diclofenac on the tendon after runs for the inflammation piece?
Also for anyone who's dealt with PTT specifically, did you find a loading phase first then drop to oral worked better than just going straight oral from the start?

- cycles_dad: Posterior tibial is the worst, that one likes to come back every time you test it... I did injectable first for 3 weeks then went straight to oral arginate and ran it about 10 weeks total, somewhere around 500-750 mcg split am/pm. For me the tendon stayed calmer than injectable alone did, but I was also doing way less mileage those weeks so honestly hard to say what did what. I did not layer TB-500, I am a little chicken of the systemic stuff for tendons that are not totally wrecked. Topical dic
- patient_codes: Yeah posterior tibial is a stubborn one, I hear you. I tried oral BPC‑157 arginate at about 600 mcg split twice daily for eight weeks while keeping mileage under 30 miles a week and the ache stayed low, but as soon as I pushed past 35 miles it flared again. Topical diclofenac after runs did help with the swelling for me too, worth trying alongside the oral. 💪
- grinder265: Appreciate that, exactly the kind of info i was looking for. The flare past 35 miles is consistent with what injectable BPC did for me too, just delayed the return rather than fixed it. Did you layer anything else in or just oral BPC and the diclofenac? And did you take any time fully off it before pushing mileage again or just kept going straight through?
- grinder265: Yeah that tendon is sneaky, like you think you're past it and then a 20 miler on camber reminds you real quick. 10 weeks is solid, did you taper the dose at the end or just stop cold? Thats kinda my worry with oral, that i stop and the calm goes with it. Ngl the split am/pm at 500-750 sounds like a reasonable middle ground, might start there and see if i can hold it through a build week.
