Stanford put out a solid write-up on why prescribers are nervous about compounded semaglutide and tirzepatide, and it lines up with what the FDA did in late April when it proposed pulling those peptides off the 503B bulks list. The framing is fair. They talk through the real concerns around sterility, dosing accuracy, and what happens when a shortage ends but the gray market keeps moving.
What I keep coming back to is how much of this could have been avoided with a $3 generic. Most people using compounded GLP-1s are not chasing boutique dosing or weird stacks. They just want the drug at a price they can actually afford. When the official channel costs ten times more, people find another way, and that is on the system, not the patients.
