Hey all, I finally decided to try a combo after months of nagging rotator cuff pain that just wouldn’t chill out with physio and ibuprofen. I started BPC‑157 sub‑Q at about 250 mcg daily, split into two injections around my shoulder (morning and evening) right after my PT stretches. A few days in I added oral Pentosan Polysulfate – 100 mg three times a day – mostly because I read it can boost cartilage proteoglycans and seemed low risk.
For the first week I felt a slight warm sensation in the joint after the BPC‑157 shots, but my range of motion improved a bit and the ache was less sharp during overhead lifts. By week three the night‑time soreness was almost gone and I could do light dumbbell work without that “creak” I always heard. I did get a mild headache once, probably from the Pentosan, but it cleared up.

After the soreness cleared I stopped the BPC‑157 at week 4 and just kept the oral. I did start logging my external rotation angle with a goniometer (got up to ~70° from ~55° pre‑start) and used the same PT stretch routine each day, so the numbers backed up the “feel” improvements. 👀
