CagriSema data actually looking better than tirz, anyone watching this one

So I've been following CagriSema for a while now and the dual-mechanism thing is what got me interested. Amylin plus GLP-1 hitting two different satiety pathways at once just makes sense to me on paper. Cagrilintide goes through the area postrema and sema does its usual GLP-1 thing, so theoretically you get appetite control from two angles instead of one.

What I'm curious about is whether anyone here has actually tried the combo or seen real-world numbers on it. The trial data shows weight loss that seems to push past what tirz alone does, but trials are trials. I'm more interested in how people are tolerating the nausea profile since hitting two pathways at once sounds like a rough first few weeks.

Also wondering how it compares to the Apollo Stack approach where you're stacking sema with 5-Amino-1MQ and MOTS-c for a metabolic boost. Different philosophy I guess, one is doubling down on appetite pathways and the other is adding cellular metabolism stuff on top.

Is anyone tracking HRV or resting metabolic rate on CagriSema? I want to know if the amylin piece preserves lean mass better than plain GLP-1 since that has been my main complaint with tirz 😅

  • leah_vo2max: The lean mass thing is exactly what keeps me from going harder on GLP-1 stuff, to be fair. I tracked HRV and RMR pretty closely on tirz and both dipped even with high protein and resistance training, so the amylin angle is interesting to me too. But I haven't tried CagriSema directly, just been reading the trial data and trying to figure out if the dual pathway actually spares muscle or if it just suppresses appetite so hard you undereat and lose more anyway.   • nomad_brandon: Yeah that lean mass question is the one i keep circling back to too, for what it is worth. I havent seen anything on cagrilintide solo, it seems like every paper pairs it with sema. Might be worth digging through the cagri monotherapy trials if those exist.
  • tiredmira: About the monotherapy trials, I didn't even think to look for those separately. Basically every paper does pair it with sema, so you're right that lean mass effect on its own is hard to pin down. If cagrilintide alone shows any meaningful body comp shift that would tell us a lot about which mechanism is doing what.
  • tiredmira: Honestly no, I haven't pulled apart the cagrilintide monotherapy data on lean mass specifically, I just assumed it tracks with the combo numbers because of the mechanism. Good callout though, I'll dig into it this week. The HRV dip on tirz is consistent with what I've seen too, which is part of why the amylin piece caught my attention. If cagrilintide alone spares muscle then the combo is doing two useful things, not just adding more suppression on top.
