Tesamorelin plus exercise trial, finally something on body comp beyond HIV lipo

New clinical trial on ClinicalTrials.gov looking at tesamorelin as an adjunct to exercise, not just for HIV-related lipodystrophy this time. That is the interesting bit for me, since most of the data I have seen is in that specific population, and we keep extrapolating to general fat loss or recomposition.

Anecdotally, I have run tesamorelin alongside CJC/ipam and the visceral fat change was the most noticeable thing, but I could not tell you how much was the peptide versus the training and diet being dialled in. Hard to separate, na?

What I want to see is the actual readouts. IGF-1 response, VAT on imaging, and whether the exercise group actually does meaningfully better than tesamorelin alone. Anyone here following this trial or planning to apply once they open enrolment?

  • tara_n: Yeah exact same boat as you, that is why I am holding off on any strong claims till we get the actual readouts. An IGF-1 baseline plus follow-up would be the minimum I want to see, otherwise we are just guessing again, no? Curious though, when you ran CJC/ipam alongside it, what was your rough IGF-1 response? Mine crept up but nothing crazy, so I keep wondering if the visceral change is just GHRH/GH pulsatility doing its thing or something more specific to tesamorelin itself.
  • aspiring_student: Yeah man exactly this, the IGF-1 and a proper VAT measurement would tell us so much more than just waist size no? Like you said, diet and training were already dialled in for both of us, so it becomes basically impossible to know what did what. I am hoping they publish the visceral adipose numbers separately from subcutaneous, because that is where I felt the change myself, not so much in the mirror honestly. Did you get any bloods done during your run?
  • tara_n: Yeah VAT separated from subcutaneous is what I am waiting for too, that is the whole point. I did get bloods yes, IGF-1 went up decently but nothing crazy, maybe within the higher end of normal for my age. Liver markers were fine. Antecdoctally the waist measurement moved more than the scale, which lines up with visceral being the target.
