Pentosan Polysulfate oral vs subcutaneous for knee cartilage support – any dosing tips around rehab?

Hey folks, I’ve been dealing with some lingering cartilage irritation in my left knee after a long trail run last month. I’ve read that pentosan polysulfate can help with proteoglycan synthesis and maybe slow cartilage breakdown, but I’m not sure how to time it with my PT sessions. Has anyone tried the oral 100mg three times a day versus the weekly subcutaneous injections (2-3mg/kg) while doing eccentric loading or blood flow restriction work?
I’m curious if the oral route gives steadier levels that line up better with daily rehab, or if the injections provide a stronger peak that pairs with harder sessions. Also, any noticed GI upset or bruising that messed with your training consistency? Just looking for real‑world anecdotes before I decide which route to try.
Ngl, I’m leaning toward subcutaneous because of the half‑life, but the oral schedule seems simpler to stick with while traveling for races. Thanks for any input.

Anecdotally, oral PPS at 100mg TID gives you steady trough levels which could be good for daily PT consistency, but GI upset is pretty common especially with longer runs at that dose. SubQ bypasses first-pass metabolism so you're looking at better bioavailability per mg. The bruising thing is real though, especially if you're doing BFR work on
- formernico: Their note about splitting subQ days away from the hardest PT sessions matches what I tracked on mine. I kept my injection for the evening after BFR and the bruising was way less than when I tried it the morning of. For what it is worth, my HRV dipped a little on injection day too.
