I just read the phase‑2 study published in Osteoarthritis and Cartilage where they gave 100 mg of oral pentosan polysulfate three times a day to 68 patients with mild‑to‑moderate knee OA for 12 weeks. The authors reported a modest but statistically significant reduction in WOMAC pain scores and, more interestingly, MRI‑based cartilage thickness was preserved compared to placebo.
For me the claim of “disease‑modifying” feels like a stretch. In my own experience with joint aches, I’ve tried a short course of oral pentosan (100 mg tid) after a bad hike and noticed a slight dip in soreness the next day, but it could’ve been just less swelling from the usual rest. The drug’s anti‑coagulant vibe also makes me nervous about bruising, especially since I still do occasional heavy lifting.
Do any of you use the oral route long‑term, or stick with the weekly SC injections? I’m curious if the added convenience outweighs the bleeding risk and whether anyone has tracked actual cartilage changes on imaging.
