Tesofensine 0.25 mg start – appetite crash, jittery nights, and HRV dip

I finally gave tesofensine a try after hearing about the appetite knock‑down. 25 mg once a day, mixed in a bit of water and took it with breakfast. 9 lb on the scale.

The downside hit around night three: dry mouth, a racing heart and my smartwatch showed HRV dropping from a baseline of 58 ms to the low 40s. I also noticed a mild insomnia, waking up 2–3 times a night. By day five the dry mouth eased but the jittery feeling stayed.

2 lb after a week despite cutting carbs. 25 mg dose? 125 mg help, or just ride it out?

The jitter and dry mouth usually ease if you keep the dose low and stay hydrated, but I found splitting it, half with breakfast, half with lunch, cut the nighttime racing feeling a bit. 25 mg yet; the extra weight loss was modest and the side‑effects started to outweigh the benefit for me. How’s your blood pressure looking? Might be worth checking that
  • amber464: My blood pressure's been a bit elevated, around 125/85, which is higher than my usual 115/75, imo that's probably due to the tesofensine, so i'll keep an eye on that, and i like your idea of splitting the dose, i might try that to see if it helps with the jittery nights, did you notice any difference in appetite suppression when you split the dose?
