Experiencing Pentosan Polysulfate for Knee Osteoarthritis – Any Others Tried?

Posted by trevor_recovery in Healing & Recovery - 1 points, 2 comments.

Hi folks, I’ve been dealing with knee pain for about two years, mostly from osteoarthritis. I started taking 100 mg of Pentosan Polysulfate orally three times a day, as suggested on the product page, after looking at a few community posts that mentioned it helped with joint cartilage support. After six weeks, I noticed my pain during walking decreased a bit, and the stiffness in the morning lessened. I also felt a slight improvement in my ability to do light jogging without the sharp ache that used to come on after a few minutes.

I’m wondering if anyone has used it for more severe OA or combined it with topical diclofenac gel for a tighter dose on the joint itself. I’ve read that Pentosan can stimulate proteoglycan synthesis and even reduce MMP activity, which sounds promising. Does anyone have any advice on timing or whether adding a topical NSAID might interfere or help? I’m still in the early stages, so I’d appreciate any practical tips or cautionary notes from those who’ve been in this situation. Thanks!

  • retired_cooks: I have not used pentosan, but i always worry about the long term safety of these kinds of compounds when the data is mostly based on animal studies. Basically, it sounds like you are feeling some relief, but it could just be placebo or the natural wax and wane of osteoarthritis. Regarding the diclofenac gel, it is generally a local effect, so it should not interfere with the systemic action of the pentosan. However, i would ask a doctor if mixing an nsaid with this particular compound is safe f
  • trevor_recovery: Thanks for the reality‑check, retired_cooks. I did get a basic CMP and liver enzymes before starting, all normal, and plan a repeat in three months. I’ll keep the diclofenac gel on the knee only in the evenings to avoid stomach irritation. Do you think adding a low‑dose oral NSAID would add any benefit, or is it better to stay with just the topical? Ngl.
