Compounded Semaglutide Injection Site Reactions: My Experience and Mitigation Tips

25 mg weekly three months ago. The first few injections were a rough ride – a sharp pinch, then a bruise the next day that lingered for a week. I noticed the pain rating rose to 7‑8 on a 10‑point scale, and HRV dipped a couple minutes after injection.
After reading about injection‑site inflammation, I switched to a 31‑g needle and started using a small warm compress for 10 minutes post‑dose. I also rotated sites between thigh, abdomen, and upper arm. Within the next two weeks the bruises were almost invisible and the pain dropped to a 2‑3.
I tracked the soreness in my daily log and saw that the lower pain correlates with better HRV readings. I’m still watching for any redness or swelling beyond 24 hours and plan to test a lower dose if pain recurs. Anyone else have a protocol that worked for you?

- brandon245: Not gonna lie, the first few shots of semag spur my skin like a bad cut. I went from a 27‑g to a 31‑g and started doing a quick 5‑minute cool compress instead of warm, because heat seemed to keep the redness in for a day. I also tried angling the needle at 45 degrees and not pulling the serialization on the skin, which cut the bruising in half for me. I still keep a log of pain and HRV, and I noticed that the lower the pain the steadier the HRV. Have you tried cooling it or a different angle?
- amber464: შThanks for the tip, brandon. I stuck with the 31‑g but always warmed the area. I’ve never angled the needle, sowam I’ll try the 45‑degree approach and see if it cuts bruises. Cool compress instead of warm might help with the redness you mentioned. I’ll log pain and HRV to compare twice a week.
- grace_sleepnerd: I’ve tried the cool compress too, and I did angle it 45° just like you said. For me the cool thing reduced the redness more quickly, but the angle helped keep the bruise smaller. I keep a log and haven’t seen any HRV change, but the pain’s definitely lower. Glad we’re matching up.
- amber464: Cool compress 45° works, that’s good to hear. I’m still doing the 10‑minute warm at first but will try the cool option after the injection to see if it speeds redness resolution. I’ve been checking HRV about two minutes post‑dose, so maybe that’s why I see a dip. What’s the time frame you’re logging HRV QUIZ? That might explain the difference.
