ARA‑290 for chronic neuropathic pain – my short run‑in

I gave ARA‑290 a try last month after a long period of restless leg syndrome and occasional shooting pains in my lower back that kept me up more than once. I started with the recommended 1 mg injection daily, mimicking the dosing that came up in a few online Span forums. After a couple of days the injection site was a bit sore but nothing dramatic.

By the third week I noticed that the sharp stabbing felt more like a dull ache and I could sleep eight straight hours for the first time in months. I kept my CBCs on the side and a quick hematocrit check came back in the normal range – no red‑cell bump. I didn’t see any headaches or other side effects that people usually mention.

Anyone else in the group using it for neuropathic pain or inflammation? How did you handle dosing and what did your blood work show?

  • hank_m: Tbh i’ve dabbled with ARA‑290 for a couple of spasms in my knees, but not for back pain, so i’m not a direct comparison. I stuck to 1 mg daily for about six weeks and the injection site was mildly sore for two days, then settled. My sleep got a little better, but i still woke up twice a night from a tingling in my toes. I didn’t run any labs, so i’m not sure about any blood changes – i’ve seen people talk about a rise in platelets, but i never checked that. I’m curious how others are dosing if
  • hank_m: Tbh your run feels pretty solid, especially the sleep win, which is a big deal for a lot of us. I’ve put ARA‑290 on the back burner too, mostly for joint aches, and saw the sharpness soften after about 3‑4 weeks, kinda like your “stabbing to dull” shift. I stuck to 1 mg daily, but I’d do a weekly CBC just in case, no red‑cell spike for me, but I still keep an eye on the bloodwork. Any of you seen changes in inflammatory processes or any odd temp hikes?
  • aspiring_trailrun: I’ve seen the same thing. 1mg daily for six weeks let my sharp pain mellow to a dull ache and the toe tingling lessened. I did a quick full blood count after the cycle – platelets were fine. Have you tried any baseline inflammation markers?
  • aspiring_trailrun: Sounds spot on. For my own run I kept a weekly CBC and haemoglobin and didn't spot any red‑cell surge or temperature spikes. The joint pain dulled after a month and I felt a mild drop in morning stiffness, but the routine bloodwork stayed within normal limits. Have you noticed any subtle changes in CRP or ESR?
  • liam_sleepnerd: Tbh I ran 1 mg daily for about five weeks on knee spasms and back pain, and the stabbing did mellow into a dull ache. I kept a simple pain diary and it showed a gradual drop in intensity. My platelet count stayed around 300 k, so I didn’t see a spike. 5 mg twice a day when the tingling got worse, and it seemed to keep the toe tingling down. Does anyone else notice a shift in sharp versus dull pain when they tweak the dose?
  • greg208: Not gonna lie, I've had no weird temperature spikes, but I did notice the CRP dip after a month, which you might catch on a routine check. I still keep a finger on the pulse, just in case. Have you noticed any swelling in the knees or shoulders?
  • scott_l: Thanks for the CRP note, haven’t tracked that yet, just going by how I feel. No swelling in knees or shoulders, though I’ve heard others mention joint stuff with bigger doses. You on a specific protocol or just going off anecdotes? Curious if the CRP drop lines up with when the pain eased for you.
  • scott_l: The 5mg twice daily worked for your tingling; did you tweak timing (like morning/night) or notice any correlation with physical activity? Also, your platelet count at 300k is lower than my 370k, any side effect correlation there?
  • scott_l: I didn’t track CRP or ESR, just general blood work, but your joint pain improvement aligns with what I felt. Curious if you tried adjusting the dose after the first month or stuck with 1mg?
  • scott_l: You two had similar pain shifts, mine softened after three weeks instead of six. I didn’t test inflammation markers specifically, just CBC, which stayed normal. Did you change anything else in your routine during that six-week window? My schedule stayed the same, so not sure if that’s a factor.
  • scott_l: I didn't notice any temp hikes or feeling feverish, just the usual stuff. It is interesting you saw that shift around the same 3 to 4 week mark because that is exactly when the lower back pain finally calmed down for me. I only did a quick check on my hematocrit, but doing a full weekly CBC sounds a bit overkill for my taste unless you are seeing something weird.
  • scott_l: I kept the 1 mg daily for four weeks total, then tapered to every other day for two more weeks before stopping. My CBC stayed flat, no platelet shift. Did you ever try splitting the dose to see if the toe tingling lessens?
