Joint pain and hot flashes on Anastrozole – any tips?

25 mg three times a week about six months ago to keep my estrogen up in check while on testosterone. Halfway through, I noticed my knees and lower back started hurting a lot, especially after a long shift or a hard training session. The pain was a dull ache that turned sharp when I moved too fast.
I also began having sudden hot flashes, usually at night, which made it hard to get a good night’s sleep. I’ve read in a few threads that joint pain and hot flashes are common side‑effects. I tried taking a multivitamin with magnesium to see if it helped the aches, and I’ve been keeping a water‑rich, anti‑inflammatory diet and doing light mobility work.
The pain is still there, but it’s less intense when I keep my joints moving, and the hot flashes have reduced. Anyone else had these effects? How did you manage or mitigate them?

- ivy_mobility: I’ve been on a low dose of anastrozole for a few months while trying to keep my estradiol in range during a testosterone cycle. Around week eight I started getting a dull ache in my knees and lower back after long shifts, and I also woke up a couple of times a night feeling hot and sweaty. I tracked my sleep HRV and noticed the hot flashes correlated with poorer recovery scores. Cutting the dose to half the frequency helped the joint pain drop noticeably, and adding a daily omega‑3 supplement s
- greg208: Thanks ivy_mobility for the heads up – I did cut the dose to 12 mg twice a week after the joint issue hit and it did ease the ache a bit. Your omega‑3 trick sounds solid; I’ve been on a 1000‑mg fish oil daily but hadn’t measured its effect. What dose were you using and did you see a change in your HRV after that? Also, have you any idea if the hot‑sweats were linked to your estradiol dipping?
- student_rational: I totally get grunting knees after a long shift. I cut my anastrozole to every other day too and saw the aches ease a bit.Pendulum of omega‑3s helped my hot flashes too – maybe it’s a gut‑to‑brain thing. Always keep an eye on your HRV, though.
- greg208: That every‑other‑day tweak sounds spot on – I’ll give it a shot next week and see if the knee ache dulls. Omega‑3s for hot flashes is a solid idea; I’ve only had a few grams a day, but it sounds worth increasing. I’ll start logging HRV on my phone to spot any shifts with the new dosing. Will you track HRV too? It might help us see what’s working.
