Cagrilintide (Cagrilintide (AM833))

Category: Weight Management. Status: Investigational (Phase III).

A long-acting amylin analog being investigated for obesity, both on its own and in a fixed-ratio combination with semaglutide (CagriSema). Amylin is a hormone co-secreted with insulin that promotes satiety and slows gastric emptying.

How it works

Acts as a dual amylin and calcitonin receptor agonist. It engages satiety centers in the area postrema and hindbrain, reducing food intake and slowing gastric emptying through a pathway distinct from GLP-1.

Key facts

  • Molecular weight: ~3963 Da
  • Half-life: ~7-8 days
  • Bioavailability: High via subcutaneous injection
  • Storage: Lyophilized: refrigerate. Reconstituted: 2-8°C, use within ~30 days.

Dosing overview

  • Typical dose: 0.3-2.4 mg (titrated)
  • Frequency: Once weekly
  • Duration: Ongoing while needed
  • Route: Subcutaneous injection

Protocol notes

  • Trials titrate slowly from a low dose (e.g. ~0.3 mg) upward over several weeks to limit nausea.
  • Once-weekly dosing is enabled by the long half-life; a fixed weekday is typical.
  • Often studied alongside semaglutide (CagriSema) to combine amylin and GLP-1 satiety pathways.

Reported benefits

  • Appetite suppression and increased satiety
  • Meaningful weight loss
  • Complements GLP-1 agonists (additive effect)
  • Slows gastric emptying

Possible side effects

  • Nausea (dose-dependent)
  • Reduced appetite
  • Injection site reactions
  • Constipation or GI upset

Research

  • Cagrilintide once weekly for weight management (2021): Phase 2 trial showed dose-dependent weight loss up to ~10.8% at 26 weeks vs ~3% placebo.
  • CagriSema (cagrilintide + semaglutide) in obesity (2023): Combination achieved roughly 15-17% weight loss, exceeding either agent alone.

  • steady_state_stan rated it 5/5 - Steady progress without the usual hunger: Been running Cagrilintide for about 4 months now and the appetite suppression is real. Not that awful "I cannot eat" feeling, more like the constant food noise just goes quiet. I actually forget about snacking, which never happened before. The first week had some mild nausea, especially after eating too fast or too greasy. Learned to eat slower and smaller portions and it resolved. Energy levels are good, no crashes or fatigue that some people report. Down about 9% body weight with no muscle loss that I can detect. Still hitting my lifts fine. The consistency is what impressed me most, no pl
  • biohack_bree rated it 5/5 - Finally broke through my plateau: Skeptical going in because I have been burned by hype before. Cagrilintide actually delivered though. The appetite reduction is subtle but persistent, you just stop thinking about food between meals. First two weeks were rough with some nausea and low energy, but I stuck with the low starting dose and let my body adapt. By week 3 I was feeling normal with clearly reduced appetite. By month 2, the results on the scale were undeniable. Key advice: start at the lowest dose, do not rush titration, eat protein-forward, and stay hydrated. The compound does the heavy lifting on appetite, you just n
  • protocol_pete rated it 5/5 - Down 27 lbs with minimal sides: I have tried a lot of weight management approaches over the years and Cagrilintide is the first that actually changed my relationship with food rather than just white-knuckling through hunger. The mental shift happened around week 2. Side effects were manageable. Some GI upset early on that settled by week 3. Mild constipation that fiber and water fixed. Nothing that made me want to stop. Results have been steady and sustainable. Losing about 1-2 lbs per week without feeling deprived. My energy is actually better than before, probably because I am not on the blood sugar roller coaster anymor

  • megan302 rated it 4/5 - quieter appetite than semaglutide, but a little too sticky: i ran cagrilintide for about 3 weeks while i was trying to get my eating back under control after a sloppy stretch of travel and too many late dinners here on the west coast. for me, it did blunt hunger pretty clearly, and i lost about 3 lbs. i actually preferred semaglutide for this goal, though, because cagrilintide felt a little more like my stomach was just sitting there. still, i’d give it 4 stars for appetite control.
